Gait apraxia is the inability to initiate and perform normal walking despite having normal muscle strength, normal sensation, and no significant cerebellar or extrapyramidal deficit. The patient has difficulty translating the intention to walk into coordinated motor action. It is also called: ๐Ÿ“Œ Frontal gait disorder ๐Ÿ“Œ Magnetic gait ๐Ÿ“Œ Bruns' ataxia ๐Ÿ’ข Pathophysiology The problem is not weakness, but a failure of motor planning for gait. Damage occurs in the frontal lobe gait center and its connections: ๐Ÿ“ŒSupplementary motor area ๐Ÿ“ŒMedial frontal cortex ๐Ÿ“ŒFrontopontocerebellar pathways ๐Ÿ“ŒBasal ganglia-frontal circuits These pathways normally initiate and coordinate stepping. ๐Ÿ’ขClinical Featurof ๐Ÿ“Œ Difficulty initiating walking ("feet seem stuck to the floor") ๐Ÿ“Œ Short, shuffling steps ๐Ÿ“Œ Broad-based gait ๐Ÿ“Œ Magnetic gait: feet appear glued to the floor and are lifted with difficulty ๐Ÿ“Œ Turning is difficult and slow (multiple small steps) ๐Ÿ“Œ Frequent falls ๐Ÿ“Œ Upper limb movements are relatively preserved ๐Ÿ“Œ Muscle strength is usually normal ๐Ÿ“Œ Sensation is normal ๐Ÿ“Œ No cerebellar signs ๐Ÿ’ขCommon Causes ๐Ÿ“Œ Normal pressure hydrocephalus (NPH) (classic cause) ๐Ÿ“Œ Frontal lobe stroke ๐Ÿ“Œ Frontal lobe tumors ๐Ÿ“Œ Advanced vascular dementia ๐Ÿ“Œ Traumatic brain injury involving frontal lobes ๐Ÿ’ขExamination Findings Ask the patient to: ๐Ÿ“Œ Stand up ๐Ÿ“Œ Start walking ๐Ÿ“Œ Turn around ๐Ÿ“Œ Walk through a doorway ๐Ÿ’ขTypical findings: ๐Ÿ“Œ Hesitates before taking the first step ๐Ÿ“Œ Feet appear "magnetically" attached to the floor ๐Ÿ“Œ Small shuffling steps ๐Ÿ“Œ Difficulty turning ๐Ÿ’ข SBA A 72-year-old man has difficulty initiating walking.
