Topic: Dosing time of Amlodipine, Cilnidipine & Telmisartan 🔴 Cilnidipine Peak effect: 2-4 hours Duration: 24 hours তারমানে কেউ যদি Cilnidipine খায় তাহলে খাওয়ার 2-4 hours পর তার BP সবচেয়ে বেশি কম থাকবে। 🔷 Best time to take 📌 Usually morning after breakfast 📌 Controls daytime sympathetic activity. 📌 Most clinical trials used morning administration. 📌 if BP is predominantly elevated at night or early morning, then prefer night dosing. 🔴 Amlodipine Peak effect: 6-12 hours Duration: 24 hours plus তারমানে কেউ যদি Amlodipine খায় তাহলে খাওয়ার 6-12 hours পর তার BP সবচেয়ে বেশি কম থাকবে। 🔷 Best time to take 📌 At morning or evening - once daily. 📌 Either is acceptable because of its very long half-life (30-50 h). 📌 If BP is uncontrolled during early morning (morning surge),then prefer night dosing. 📌 If day time dizziness occurs, taking it at bedtime may improve tolerance. 📌 Missing dose: Missing one dose causes only a small change in BP. 🔴 Telmisartan Peak effect: 3-5 hours Duration: 24 hours plus তারমানে কেউ যদি Amlodipine খায় তাহলে খাওয়ার 3-5 hours পর তার BP সবচেয়ে বেশি কম থাকবে। 🔷 Best time to take 📌 Morning or bedtime 📌 Many specialists prefer bedtime dosing if morning BP surge, nocturnal hypertension, Diabetes, CKD 📌 Otherwise morning dose is preferable. 🔴 Note : Maximum antihypertensive effect of all three drugs usually develops over 2-4 weeks, even though BP starts falling after the first dose. 🔴 If all three are prescribed together 🔷 At morning 📌 Cilnidipine 📌 Telmisartan 🔷 At night 📌 Amlodipine 🔴 Note: However, fixed-dose combination (e.g., Telmisartan + Amlodipine) is commonly taken once daily at the same time each day, and consistency is more important than the exact clock time. 🔴 Can you prescribe all these 3 drugs at a single time? Yes. All three drugs can be given together in the evening if there is a clinical reason and the patient tolerates them well. Because all these three are once-daily, long-acting medications, evening dosing generally provides effective 24-hour blood pressure control. 🔷 Advantages of evening dosing 📌 Better control of the early morning blood pressure surge. 📌 Convenient if the patient is more adherent to taking medicines at night. . 🔷 Potential disadvantages of evening dosing 📌 If the patient is elderly, volume-depleted, or prone to postural hypotension, taking all three together may increase the risk of dizziness or falls, especially if they get up during the night. 📌 Blood pressure should be monitored after starting or increasing doses. 🔴 Note: Using both amlodipine and cilnidipine together is not routine practice. They are both dihydropyridine calcium channel blockers. Combining them usually provides little additional benefit while increasing the risk of adverse effects such as hypotension and peripheral edema. In most patients, one CCB plus an ARB is preferred.
