Comparative pharmacology of Thiazide as an antihypertensive

🔴Antihypertensive হিসেবে indapamide/Chlorthalidone is better than Hydrochlorothiazide (HCTZ). কারণ, 📌 এদের Duration of action বেশি। Chlorthalidone এর 48-72 h. Indapamide, এর 24 h. 📌 এরা দুজনেই more potent than HCTZ 📌 দুজনেই Nocturnal BP control করতে পারে তারমানে Indapamide and Chlorthalidone দুজনেই HCTZ এর চেয়ে Better. এখন আসি, Indapamide আর Chlorthalidone এর মধ্যে কে better? Antihypertensive effect সবচেয়ে বেশি Chlorthalidone এর। তবে এর Metabolic side effects গুলো বেশি। যেমন: 📌Hyponatremia 📌Hypokalemia 📌Hyperglycemia (not suitable in DM patient) 📌Hyperuricemia কিন্তু Indapamide এর ক্ষেত্রে উপরের side effect গুলো অনেক কম হয়। তাই এটা Better tolerated than Chlorthalidone. 🔴 Summary: সবচেয়ে ভালো BP কমাতে পারে Chlorthalidone. তবে BP মোটামুটি ভালোই কমায় এবং সাথে Better tolerated হলো, Indapamide. So, সবকিছু মিলিয়ে তিনজনের মধ্যে আজকের Winner হলো "Indapamide" Thiazide এর দিন শেষ। That means, আজকে থেকে Antihypertensive হিসেবে আপনি Add on therapy হিসেবে Indapamide কে prefer করবে। কিন্তু এতেও যদি BP control না হয়, Resistant HTN develop করে তখন আপনি Chlorthalidone এ switch করবেন। 🎓 Feel the MEDICINE course Dr. Muhammad Mosleh Uddin.
