Man can be both accidental and main host for Leishmania

Final prof VIVA | Medicine 🔴 আচ্ছা বলো, Visceral Leishmaniasis-এ মানুষ কি Main host নাকি Accidental host? - স্যার, দুটোই হতে পারে। এটি Leishmania-এর life cycle-এর type এর উপর নির্ভর করছে। Anthroponotic life cycle এ মানুষ হলো main host/reservoir. আর Zoonotic life cycle এ মানুষ হলো accidental host/reservoir. চলুন, একটু deatails এ বলি, ইনশা আল্লাহ। Leishmania protozoa এর ২ ধরণের life cycle আছে। 1. Anthroponotic life cycle 2. Zoonotic life cycle 🔷 Anthroponotic life cycle: Human → Sandfly → Human এখানে মানুষই হলো Main host/reservoir. অর্থাৎ, Leishmania protozoa র life cycle maintain করতে এখানে Human মানুষই গুরুত্বপূর্ণ ভূমিকা রাখে। এই life cycle কোন ধরণের Leishmania species এ দেখা যায়? এটা Leishmania donovani species এ দেখা যায়। আর এটাই বাংলাদেশ, ভারত, নেপালে বেশি হয়। 🔷 Zoonotic life cycle: Dog → Sandfly → Human এখানে কুকুর বা Canine animal হলো main host/reservoir এবং মানুষ হলো accidental (incidental) host. কারণ এখানে Leishmania protozoa র life cycle maintain করতে মানুষ জরুরি না। মানুষ ছাড়াও এই life cycle continue হতে পারে। এই life cycle কোন ধরণের Leishmania species এ দেখা যায়? এটা Leishmania infantum and Leishmania chagasi species এ দেখা যায়। 🔴 তাহলে মনে রাখবেন, Visceral leishmaniasis এ মানুষ সবসময় accidental host - এই কথাটি ভুল। 📌 L. donovani → Human are main host/reservoir 📌 L. infantum → Human = Human are accidental host বাংলাদেশে Visceral Leishmaniasis-এর main pathogen হলো Leishmania donovani, তাই আমাদের দেশে মানুষই প্রধান reservoir/host। Feel the MEDICINE Dr. Muhammad Mosleh Uddin MBBS (CMC), FCPS P1 (Cardiology)
