🔴 ARB + CCB combination therapy is Superior to High dose of ARB alone to prevent Cardiovascular events in CKD Based on current research evidence, for patients with Chronic Kidney Disease (CKD), using a standard dose of an ARB combined with a Calcium Channel Blocker (CCB) is superior to using the highest dose of an ARB alone for preventing cardiovascular events. This conclusion is primarily supported by the landmark OSCAR study: · Better cardiovascular protection: Among CKD patients (eGFR < 60 ml/min/1.73m²), the combination therapy group experienced significantly fewer cardiovascular events (16 cases) compared to the high-dose ARB group (30 cases). The high-dose ARB group had a 2.25-fold higher risk of major events. · Better blood pressure control: The combination therapy group also achieved better blood pressure reduction than the high-dose ARB monotherapy. In summary, for hypertensive CKD patients, the ARB + CCB combination has stronger evidence for cardiovascular protection and blood pressure control than simply up-titrating the ARB to its maximum dose.
