কেন আমরা Urticaria বা allergic reaction এ Antihistamine and Montelucast দুটোই prescribe করি?

🔴 Urticaria বা Allergic reaction এ কেন আমরা Antihistamine and Montelucast দুটোই দেই? প্রথমেই দেখি যে, macrophage, mast cell এবং eosinophil এই তিনটা সেল during inflammation একে অপরের সাথে কিভাবে interaction করে। যখনই macrophage কোনো একটা foreign particles, pathogen অথবা damaged tissue particle engulf করে, তখন macrophage কি release করে? Interleukin-1, 6, TNF-alpha release করে। এর মধ্যে interleukin-1 এটা কাকে activate করে? এটা mast cell কে activate করে। Allergic reaction বা Type-1 hypersensitivity এর ক্ষেত্রে Mast cells এর cell membrane এ attached থাকা IgE, allergen particles দ্বারা triggered হলেও Mast cells activate হয়ে যায়। এক্ষেত্রে Macrophage এর প্রয়োজন নেই। Mast cell activates হলে কি release করে? Mast cell তখন release করে, 📌 Histamine 📌 Eosinophil chemotactic factor (ECF) 📌 Leukotrienes (LT B4) এই Lt B4 এবং Eosinophil chemotactic factors eosinophil হলো chemokines for Eosinophils. এরা Eosinophils কে ডেকে আনে at the site of inflammation. Eosinophil এসে, সে আবার নিজে আরো বেশি বেশি leukotrienes release করতে থাকে. ফলে তখন এই leukotrienes আরো বেশি বেশি Eosinophils কে ডেকে আনে। That means আমরা বুঝলাম যে প্রথমে macrophage activated হচ্ছে by phagocytosis, সে mast cell কে activate করছে, mast cell তারপর eosinophil কে ডেকে আনছে। So, এখানে তিনটা immune cells ই আসলে involved. এখন দেখা যাচ্ছে, according to study, allergic reaction বা helminthic infestation এর সময় অর্থাৎ type-1 and type 2 hypersensitivity যেগুলোতে হয়, সেগুলোতে Mast cells and Eosinophils predominantly involved থাকে। এই কারণেই Urticaria তে আমাদেরকে কি করতে হয়? আমাদেরকে ২ টা cells কেই stable করতে হবে। ১. Mast cells ২.
